Outcome Statement:

The purpose of this session was to prepare healthcare quality improvement professionals to implement effective communication and coordination of care strategies by exploring promising practices that address community and social support needs. This experience helped participants demonstrate and promote successful communication and coordination of care practices and identified opportunities for improvement, all of which promote advances in care that impact the Medicare beneficiaries served by the work of the QIO Program.

Event Description:

This session was a follow-up event to the May 3rd National LAN Call: “Advancing Effective Communication and Coordination of Care.” It featured speakers from the Southwestern Pennsylvania Area Agency on Aging, Primaris, and Telligen. This call was open to QIN-QIOs, provider, partners, and beneficiaries.

The purpose of this call was to follow-up with how participants left the May 3rd session in action and to generate discussion between participants and speakers on communication and coordination of care strategies and tactics.
